This Privacy Policy describes how Claimant Inc. (“we,” “us,” “our”) collects, uses, and shares information when you visit www.claimantinc.com (the “Site”) or use our services to help you prepare and submit insurance appeal materials (the “Services”).

2) What this policy covers (and what it does not)

This policy covers information we collect from you through the Site and Services, including information you submit through forms and communications with us.

Important note about HIPAA: We are not a health plan, healthcare provider, or healthcare clearinghouse. In most cases, our Services are not governed by HIPAA. That said, we treat the health and insurance information you provide as sensitive and we work to protect it. (If we ever operate as a HIPAA “business associate” under a signed contract, we will update our practices and notices accordingly.)

3) Information we collect

A. Information you provide to us

You may provide the following types of information:

  • Identifiers and contact information, such as your name, email address, phone number, mailing address, date of birth, and similar details.

  • Insurance and claims information, such as your insurer name, plan details, member or claim identifiers, billing and payment information related to the claim, denial details, and your communications with the insurer.

  • Health-related information (sensitive), to the extent it appears in materials you provide (for example, an Explanation of Benefits (EOB), denial letters, itemized bills, clinical test names, procedure codes, diagnosis codes, provider information, and dates of service).

  • Communications, such as emails/messages you send us and any information you include in them.

B. Information collected automatically

When you use the Site, we (and our service providers) may collect:

  • Device and usage information, such as IP address, browser type, pages viewed, and approximate location (inferred from IP).

  • Cookies and similar technologies used for basic site functionality, analytics, and security.

4) How we use your information

We use information we collect to:

  • Provide the Services, including reviewing the materials you provide, preparing appeal packages (letters, forms, instructions), and delivering those materials to you.

  • Communicate with you about your request, ask for missing information, and provide customer support.

  • Improve and maintain the Site and Services, including troubleshooting, analytics, and quality assurance.

  • Protect against fraud, abuse, and security incidents.

  • Comply with legal obligations and enforce our terms.

5) How we share your information

We share information in the following circumstances:

  • Service providers. We use vendors to host our Site, collect form submissions, store data, and send emails (for example, Wix and email service providers). These vendors process information on our behalf to help us run the Services.

  • With your direction. If you ask us to, we may help you submit appeal materials to your insurer or another party. Otherwise, our default approach is to provide the materials to you to submit.

  • Legal and safety reasons. We may disclose information if we believe it is necessary to comply with law, respond to lawful requests, protect rights and safety, or prevent fraud.

  • Business transfers. If we are involved in a merger, acquisition, financing, reorganization, or sale of assets, information may be transferred as part of that transaction.

No sale of sensitive health information

We do not sell your sensitive health information. We do not share your health-related information with advertisers for targeted advertising.

6) Your choices and requests

You can request to:

  • Access the information we have about you.

  • Correct inaccuracies.

  • Delete your information (subject to limited exceptions, such as security, dispute resolution, or legal compliance).

7) Data retention

We keep your information for as long as reasonably necessary to:

  • Provide the Services you requested,

  • Maintain business records,

  • Resolve disputes,

  • Comply with legal obligations, and

  • Protect our users and our business.

When we no longer need the information, we take steps to delete it or de-identify it.

8) Security

We use reasonable administrative, technical, and organizational safeguards designed to protect your information. However, no method of transmission or storage is 100% secure. Email is not an encrypted communications channel by default, so please consider what you share by email and use the most secure option available to you.

9) If there is a security incident

If we become aware of a security incident or unauthorized disclosure involving certain health information, we will evaluate our notification obligations and, when required, notify affected individuals and regulators. Depending on the circumstances and how the law applies, this may include notifications under state breach-notification laws and potentially the FTC Health Breach Notification Rule.

10) Children’s privacy

Our Site and Services are not intended for children under 13, and we do not knowingly collect personal information from children under 13. If you believe a child has provided us information, contact us and we will take steps to delete it.

11) State-specific privacy rights (U.S.)

Some U.S. states provide residents with privacy rights regarding their personal information (such as the right to access, delete, or correct certain information, and to opt out of certain types of data sharing). We provide the rights described in Section 6 to all users. If you are a resident of a state that provides additional rights and you want to make a request, contact us at hello@claimantinc.com and we will respond consistent with applicable law.

12) Changes to this policy

We may update this Privacy Policy from time to time. If we make material changes, we will update the “Last Updated” date and, where appropriate, provide additional notice on the Site.
